The AI Longevity Clinic Social Worker serves as the compassionate guide for each patient’s journey through the AI Longevity Clinic. This role bridges cutting-edge AI healthcare with the human touch by assisting patients and families with personalized navigation through diagnostics, treatment plans, wearable technologies, and wellness resources. The Social Worker ensures each patient receives seamless, respectful, and coordinated care from start to finish.

Reports to: Clinic Director / Chief Patient Officer
Key Responsibilities:
- Patient Intake & Orientation
- Welcome and onboard new patients with empathy and professionalism
- Explain clinic services, care processes, and technology use (e.g., health watch, glucose monitor)
- Care Coordination
- Schedule appointments, follow-ups, and video consultations
- Coordinate between departments (e.g., diagnostics, therapy, telehealth, and research)
- Support & Communication
- Communicate with patients and families, ensuring understanding of medical records, diagnostics, and medication instructions
- Provide language or cultural support, if needed
- Secure patient consent for family access to shared health updates
- Technology Assistance
- Help patients use AI-connected devices (e.g., wearable monitors, home oxygen devices)
- Ensure data is correctly integrated into the clinic portal
- Advocacy & Experience Oversight
- Identify patient concerns and advocate resolutions
- Guide patients in accessing community resources, wellness plans, and specialty services
- Recordkeeping & Reporting
- Document patient interactions, follow-ups, and service feedback in the clinic CRM system
- Contribute to improving patient satisfaction initiatives
Qualifications:
- Background in healthcare, social work, or patient relations (RN, LPN, MSW preferred but not required)
- Experience with patient navigation or concierge medical services a plus
- Strong empathy, communication, and organizational skills
- Tech-savvy and familiar with wearable health devices, telehealth, or EMR systems
- Bilingual (English/Spanish) preferred for South Florida region
